Do Not Send Medical Information Through This Website
The website contact form is not intended for medical records, detailed health information, emergencies, or urgent clinical communication. If you are experiencing an emergency, call 911.
This Website Privacy Policy explains how the Greatfaith Managed Care website (this "Site") handles information. It applies only to this Site. It does not describe how the practice handles protected health information in the course of care. Those practices are addressed separately in a Notice of Privacy Practices, which will be provided by the practice.
1. Scope of this policy
This policy covers general information handled through this Site, such as a message you send using the contact form and standard technical information processed when any website is visited. It does not cover information collected during medical care, by telephone, by fax, or through any other channel outside this Site.
2. Information you provide
You are not required to create an account or provide information to browse this Site. If you use the contact form, you may provide your name, phone number, email address, ZIP code, a service of interest, a preferred contact method, and a short message. Please share only what is needed to route your inquiry. The form asks you not to include medical records, diagnoses, medication lists, insurance identification numbers, or urgent medical information.
3. Information processed automatically
Like most websites, when this Site is visited, our hosting provider may process standard technical information as part of delivering the Site, which can include an IP address, browser type, device information, and the pages requested. This information is used to operate and secure the Site. This Site does not run advertising trackers, analytics tools, or third-party session-recording tools.
4. Cookies and storage
This Site's own code does not set cookies and does not use browser local storage or session storage to track you. The fonts and icons used to display the Site are served from this Site itself rather than from an outside content network, so loading a page does not send your browser's request information to a third-party font or icon provider.
5. Third-party service providers
We use a limited number of service providers to operate this Site:
- Website hosting, which serves the Site and may process standard request logs.
- An email delivery provider, used to transmit contact-form submissions to us by email when that service is enabled (see the next section).
Each provider processes information according to its own terms. We do not sell your information, and we do not share it for third-party advertising.
6. How form submissions are handled
When you submit the contact form, your message is sent to the practice so we can respond to your inquiry. When email delivery is configured, the submission is transmitted to us by email through a third-party email delivery provider. If email delivery is not available, the Site will tell you that online requests are temporarily unavailable and ask you to call or email us instead. We use contact-form information to respond to your inquiry and to follow up about the care you are asking about.
7. Website information vs. protected health information
It is important to understand the difference between three things:
- General website information handled through this Site and covered by this policy.
- Protected health information handled by the practice during care, which is governed by federal and state health-privacy law rather than by this website policy.
- The Notice of Privacy Practices, a separate document that describes how the practice uses and discloses protected health information. That notice, when finalized by the practice, governs protected health information.
Please do not use this Site or its contact form to send protected health information.
8. Information security
We take reasonable steps intended to protect information handled through this Site. However, no website or method of transmission over the internet can be guaranteed to be completely secure, and we cannot promise absolute security. Please keep this in mind when deciding what to send through the contact form.
9. Data retention
We keep contact-form information only as long as reasonably necessary to respond to and follow up on your inquiry, and as otherwise permitted or required. We do not set a fixed retention period in this policy.
10. Children's privacy
This Site is intended for a general adult audience and is not directed to children under 13. We do not knowingly collect personal information from children through this Site.
11. External links
This Site may link to third-party websites, such as government resources, for your convenience. We are not responsible for the content or privacy practices of those websites. Review their policies before providing information.
12. California privacy inquiries
If you are a California resident and have a question or request about information handled through this Site, you may contact us using the details below and we will respond as appropriate. This Site does not sell personal information.
13. Changes to this policy
We may update this policy from time to time. When we do, we will revise the "Last updated" date at the top of this page. Continued use of the Site after an update means the updated policy applies to your use of the Site.
