Care delivered at home or in a community setting can be easier to attend, more comfortable, and better coordinated with the people already involved in a patient's care. What a visit includes depends on the patient's clinical needs and provider availability.
Mobile primary care may be a good fit for patients who find travel difficult or who benefit from care at home.

A member of our care team meets the patient in their home or community setting for an evaluation based on clinical needs.

Some primary care visits and follow-ups may be available by telehealth when an in-person examination is not required. An in-person mobile visit is arranged when a hands-on assessment is needed.
