Greatfaith Managed Care is a mobile primary care practice. Our mission is to make quality care easier to reach by delivering it where patients already are.
For many patients, getting to a clinic is a real barrier. Older adults, homebound individuals, and people managing ongoing health needs often benefit from care that comes to them. Greatfaith is built around that idea: bringing care into appropriate home and community settings, and keeping everyone involved in a patient's care working together.
